We have provided the option to upload copies of your documents using the form below. Please consider using this convenient function. However, if you prefer to send us your documents via email or bring them directly to our office, please keep in mind that we cannot schedule any aircraft or instructors until you have done so.

On this form, we will ask for copies of:

• Proof of Citizenship (U.S. passport preferred)
• Government issued photo I.D.
• Proof of aircraft renter’s insurance or;
• Aircraft ownership insurance listing “Independence Aviation, LLC” as Additional Insured if we are training in your aircraft

Depending on your situation, we may also ask for:

• A copy of your FAA Airman Certificate (if you already have one)
• A copy of your FAA Medical Certificate (if you have already obtained one)
• For students joining us from abroad, your current and valid H-type, F1 or M1 Visa and any other authorization from US Customs and Immigration which allows you to receive Flight Training in the United States of America.

This form must be completed, waivers signed, and all documents received prior to scheduling any aircraft or instructors.
